Fill out this CoSAC Advising Google Form if:
-You have recently been disqualified from the College of Science
-You have recently been disqualified from San Jose State University
-You need an Open University Hold on your account removed
-You are a former student returning
-You need Early Support Advising

A CoSAC Advisor will reply to your Advising Google Form responses between 5-15 working business days after your submission date. During peak advising season, it may take closer to 15 working business days to reply.

If you would like a Zoom appointment, you will be able to indicate your preferred availability on this form.
