Please fill out a new form for each child--time stamp of first entry will be used for all of your children.  You will be notified within a week if your child has a spot in our summer program.

Enrollment Fees
The Summer Enrollment Fee is $5 non-refundable registration fee per child for new students ($25 for current students) plus one day/week’s tuition for the 6 week session.  Enrollment fees must be paid via check by the date in your enrollment confirmation email.  Emails will be received in one week.
 
The balance of the tuition will be due June 1.

Current Students--$250 total enrollment fee/child
New Students--$275 total enrollment fee/child



I agree to submit a check for the Enrollment Fees to St. John's UMC PDO in person or by mail no later than the date listed in my enrollment confirmation email.  If payment is not received I understand that my Enrollment application will not be processed. *
