Parent/Guardian Authorization
I hereby certify that I am the parent or legal guardian of the above named child and am therefore authorized to provide the releases, authorizations, and permissions stated herein. I certify that all information provided on this form is accurate and complete. I hereby give permission for the registered child to participate in any and all activities prepared by the Fairfield Foundation during the Fairfield Archaeology Summer Camp.
I agree that Fairfield Camp staff, will adopt rules for all campers' participation in the Camp and may apply these rules to the above-named child. I understand that the child will be engaging in indoor and outdoor activities, and will be interacting with other children. I understand that Fairfield Foundation cannot control all of the conditions which the child will encounter as a camper, and that some of such conditions could be considered to involve a risk.
I agree to hold harmless, indemnify and defend Fairfield Foundation, Inc., its officers, directors, and employees against any and all claims, damages, judgments, costs including reasonable attorney’s fees, which may arise in any way or be connected with the above-named child's participation in the Fairfield Archaeology Summer Camp. I understand that this release and indemnification includes any claims based on the negligence, actions or inaction of Fairfield.
I hereby indemnify, defend and hold Fairfield harmless from any claims I might have and/or the above-named child might have for personal injury and property damage resulting from the minor child’s participation in the Fairfield Archaeology Summer Camp.
