Mid-Year Report
This form can be used to request a Mid-Year Report/Transcript. Some colleges require a mid-year report to be sent on your behalf.  Check with your college to see if this is something that you need.  The basics of the mid-year report are: GPA, class rank (if we did that), and an updated transcript, which is why we will process them after Fall term grades are processed. https://www.collegetransitions.com/blog/mid-year-report/

After you complete this form you will need to go to SchoolPay and pay $2 for each report.  (Number of reports x $2). https://www.schoolpay.com/pay/for/Transcripts/SiZTWL   If you qualify for free or reduced meals you do not need to pay.  If you have any questions or concerns about your transcript request you can contact our counseling secretary, Leann Hollenbeak, at hollenbeak_l@4j.lane.edu.
