We want to know how our community is doing during this strange and difficult season. We also want to assess people's interest in and comfort with the possibility of a phased reopening.

 We know this is an overwhelming time. There are no required questions. Please answer only as many of these questions as feel comfortable and invitational to you, and skip any that don't.

We are considering a "hybrid model" of partial in-person worship with these restrictions (and others TBD): local infection rate is very low and not increasing; masks and social distancing required; limited attendance; no shared Communion or congregational singing.
