DESOD will be contacting parents in the order that registration was received to collect payment and secure spots in the summer intensive. I have read the RELEASE AND WAIVER OF LIABILITY, ASSUMPTION OF RISK; AND  INDEMNITY AGREEMENT, and I understand that I assume full responsibility for the risk of  bodily injury, illness, or death due to the negligence of released parties or otherwise. In the case of  illness including, but not limited to, MRSA, Influenza, and COVID-19, whether the infection  occurs before, during, or after participation with Diana Evans School of Dance, Inc. I understand that I assume full responsibility.   I voluntarily assume those risks and I have given up substantial rights by signing it and have  signed it freely and without any inducement or assurance of any nature and intend it to be a  complete and unconditional release of all liability to the greatest extent allowed by law. I agree that  if any portion of this Agreement is held to be invalid, the balance, notwithstanding, shall continue in  full force and effect.   *

Diana Evans School of Dance engages photographers or videographers to take photos or videos of  students during class and studio events to display in the studio, in promotional materials, in the recital program, social media, and on the website. Please indicate below your permission or declining  your permission for the above named student to be used for these purposes.  No dancer will be named in our photos or videos. *
