This form should be completed by our families for the 2021-2022 school year.  The deadline to complete this form will be Saturday, August 17, 2021.

After August 17th, if you selected virtual learning for your child, then you will be contacted by the school to setup a consultation process about virtual learning.

Those deciding to take only online classes are responsible for obtaining internet as needed.  All students will be provided a Chromebook as needed for instruction.  

Please submit one survey per child to enroll your child for the 2021-2022 school year.
