SOCk Camping Trip & LBT Tour Sign Up
DEADLINE TO SIGN UP: FRIDAY MARCH 20th (3/20/2020)

This is the form to sign-up to attend the annual SOCk camping trip and to tour the LBT on April 18th, 2020.

This trip is for OSC students, faculty and staff ONLY. If you wish to bring your family, you must contact Matt and Josh before the deadline of this form (see below). The LBT tour has a limit of 60 available people and EVERYONE who wishes to attend this tour must fill out this form. If a name is not on our list, that person will NOT be allowed to attend this trip or tour the LBT.

I understand that I must fill out this form in order to attend this trip and the LBT tour. I understand that if a name is not on the final attendance list, that person will NOT be allowed to attend this trip or tour the LBT.

I understand that if I plan to drive my own vehicle that I must receive DIRECT APPROVAL from Matthew Noyes and Josh McDonald, and that I will drive others who need rides to the campsite and back. I understand that parking at the campsites is limited.

I understand that if I wish to bring family, I will contact Matthew Noyes and Josh McDonald prior to the sign up deadline.

I understand that if I can no longer attend the trip after filling out this form, that I will contact Matthew Noyes and Josh McDonald to make my tour spot available to someone else.
