THIS FORM IS NOT FOR SPED TRANSPORTATION. Please contact the Diagnostician at your student's campus. They will send us your student's paperwork, and your Coordinator will then contact you.

You will receive an email or phone call (voicemail if no one answers, but obnly if your voicemail box is set up and/or able to receive messages) after August 3rd.

We can only transport from the student's legal physical address, or the student's grandparent's legal physical address. The address must be within their school's transportation zone. Their address must also be in their student record (TEAMS).
Solo podemos transportar desde la dirección física legal del estudiante o desde la dirección física legal de los abuelos del estudiante. La dirección debe estar dentro de la zona de transporte de su escuela. Su dirección también debe estar en su expediente estudiantil (TEAMS).


Only one (1) student can be registered on each form.  
ALL information must be filled in, or the system will not process the entry. 
An incorrect student ID will not be processed by the system. 
Duplicate entries for the same student will slow the process of registration.

IF YOUR STUDENT HAS ATHLETICS, BAND, OR OTHER ACTIVITIES, AND THEREFORE WILL NOT RIDE UNTIL THEIR SEASON IS OVER, PLEASE WAIT TO REGISTER THEM. IF YOU REGISTER YOUR STUDENT NOW, AND THEY DO NOT RIDE, THEY WILL TAKEN OFF OF THE ROUTE ROSTER AND YOU WILL NEED TO RE-REGISTER THEM WHEN THEIR SEASON ENDS. PLEASE JUST WAIT UNTIL THEIR SEASON IS ENDING.
SI SU ESTUDIANTE TIENE ATLETISMO, BANDA U OTRAS ACTIVIDADES, Y POR LO TANTO NO MONTARÁ HASTA QUE TERMINE SU TEMPORADA, ESPERE PARA REGISTRARLO. SI INSCRIBE A SU ESTUDIANTE AHORA Y NO VIAJA, SERÁ ELIMINADO DE LA LISTA DE RUTA Y DEBERÁ VOLVER A REGISTRARLO CUANDO TERMINE SU TEMPORADA. POR FAVOR, SOLO ESPERE HASTA QUE SU TEMPORADA TERMINE.

Transportation will not, in most cases, start the same day that you fill out the form. Please fill out the registration request at least three working days before the day that you will need transportation. Thank you!
