South Prairie Basketball Summer Skills Camp

This is the sign up information for basketball skills camp. To register, please fill out the following information.

Basketball skills camp every Tuesday, wed, and Thursday. Three week session or six week session. 4th-7th grade boys and girls from 11-12, and 8th-12th grade boys and girls from 12:15-1:15. Cost is $110 for 3 weeks or $200 for 6 weeks. Limited to 28 kids per session (14 kids per gym).

You can pay ahead of time through PayPal by sending it to sheila.gerding@gmail.com or bring a check written to Sheila Gerding the 1st day of camp.

I give my permission for (see below for name of player) to participate in South Prairie camps. I understand that South Prairie School and all personnel involved in the camp have no responsibility, assumes none, and does not carry any accident insurance for the benefit of the players. I release the camp, playing facilities and all others associated with this camp from all such claims on the account of any injuries, which may be sustained while attending the camp. I also understand the CampPersonnel and South Prairie school is not responsible for any medical expenses or bills incurred by my son or daughter while attending the camp. Parent Signature:
