- Provide your name, address and contact information including email and telephone number.
- Select the evening clinic you wish to attend from the list of available clinics

Site Visits
- If you would like to be considered for a site visit, please provide a brief description of the issues you are having with your building in the fields provided. Please note that site visits are on a first-come, first-served basis and are limited to 10 site visits per clinic. You will be notified by the Clinic co-sponsor the week of the clinic if you have will receive a site visit.
