CTK 2019 Summer Camp Registration
Please fill out all information completely.  Personal information will not be shared with any organizations outside of Christ the King Lutheran Church and Rainbow Trail Lutheran Camp.  You will not receive mailings from either organization based on information shared here unless you check that you would like to below.

After you complete this form, you will need to download and fill out the health information form for 2019 and return that to the CTK office, or email a scanned copy to office@ctkdenver.org.  All payments for registration can be paid by check or cash, and given to the CTK office or brought on the first day of camp on June 24th.

Agreement
Our child has permission to take part in all Summer Camp activities led by Christ the King Lutheran Church (Church) and Rainbow Trail Lutheran Camp (Camp).  We agree that the Church, Camp, and their personnel will not be held liable for accidents arising therefrom.  I give Church and Camp personnel permission to seek medical treatment for my child in case of injury or illness.  I also give permission for photos, video, and electronic images to be taken of me or my child and used by the Church or Camp for promotional purposes without compensation, inspection or approval.

An up to date health form is required for registration.  Please download from our website or pick-up a copy from the CTK office to fill out prior to the first day of camp.
