PHPA Application Form - Award for Groups
Descriptions of roles involved in the application process:
Applicant: the adult leading the application and the primary contact during the process.
Recipient(s): The group of people for whom the award would primarily benefit.
Recipient(s) music teacher: The qualified music teacher involved
Witness: Professional person able to independently vouch for the recipient(s). The witness must be willing to be independently approached by PHPA as part of the application approval process. (The music teacher may fulfil the role of witness if they are not the applicant.)
