This survey is designed for Weld RE-4 parents/guardians to help to identify needs at their school. This information will be used to create and implement an effective counseling program in order to fulfill the needs at each school, to create a safe and healthy environment for students and teachers, and to help students to become successful students and adults. School professionals will select the two highest needs from each category and will work on implementing a school-wide program that would cover the two highest needs at school.
