STEM Kit Request Form

Please use the following form to make your STEM Kit requests. All requests are met on a first come, first served basis. A full list of STEM kits descriptions can be found on https://tra.extension.colostate.edu/ - under the STEM/k-12 Tab. Please contact CSU Extension STEM Agent, Stephanie Lamm, at 970-249-3935 with additional questions.

Form may be used to request multiple STEM Kits, but only for 1 event or event series (.i.e. short classroom course, etc...). If requesting kits for different events, please fill out a separate form for each event.

Plan ahead! Make all kit requests at least 2 weeks prior to event/program!
