I understand that if approved and accepted legal pet adoption papers and a $150 adoption fee applies. I give permission to have references contacted during the adoption screening process. I acknowledge that all information on this form is true and correct. I understand that any misrepresentation of fact may result in Woodland Whiskers refusing adoption privileges to me. If my request for adoption is approved and later Woodland Whiskers discovers the above information is not correct, Woodland Whiskers reserves the right to remove the adopted cat from my home. *
