Pink Sheet - College Application Processing Form
Please use this form to inform your counselor that you have applied to colleges and need your college application materials to be sent to the school. You MUST complete this form in order for your counselor to send your transcript, letters of rec and other materials. College materials will not be sent without the completion of this form and parent acknowledgement sent.

Please allow at least 10 days for the school to process your request. Please see the deadlines below and make sure you submit this form in enough time.

Please complete this form for EACH college you applied to - so fill it out 5 times if you are applying to 5 different schools.

Application Processing Procedures

Students must submit a completed pink form for every college/university they apply to.  


Students are encouraged to ask two teachers; each teacher should be provided with a TEACHER Recommendation Form at least three weeks prior to the application due date.

Students must submit their Request for Counselor Letter of Recommendation at least three weeks prior to the application due date.  

Any potential athlete MUST submit their standardized test scores directly from the testing center to the NCAA.

Please notify the counseling office of all admissions decisions as soon as you are notified.

The Common Application FERPA waiver must be completed > fill out the Education Tab of your CommonApp > add at least one college > under that specific college click Recommenders & FERPA > complete FERPA Release Authorization, this will apply to all other colleges added to your college list.
To match your Common App & Naviance: Log in to Naviance, click the Colleges Tab > Colleges I’m applying to >  Click Match (in the red bar at the top of the page) > bar will turn green once accounts are matched.

***It is the student’s responsibility to request that the testing agency send scores to colleges***
You must go to the websites below and request your scores be sent directly to the colleges. 
BHS will NOT send your SAT/ACT/AP scores.
*Please note it takes 4-6 weeks for testing agencies to process requests.

Teacher Letters of Recommendation
In Naviance, under the “Colleges Tab”, click on “Colleges I’m Applying To”. At the bottom of the page add in the teacher(s) you would like to request letters of recommendation from.  Some schools will only accept 1 or 2 teacher letters maximum. You must list the order you would like these letters sent.  If only one letter is required, the first name listed is the one we will send. If two letters are required, then the names listed in only #1 & #2 will be sent, etc.
Please list who you asked for letter of recommendation #1, please type N/A if your college does not require letters of recommendation. *
