Registration for Medication Review
Thank you for your interest in a medication review at the Homewood CEC!  These reviews are conducted by pharmacy students under the supervision of a pharmacist.

We look forward to meeting with you and answering any medication related questions you may have.  To make the most of your visit, please plan to bring your current medication list and/or medication bottles (this includes anything you take over the counter.)  Recent paperwork from a doctor visit or hospitalization is also helpful.  If someone helps you with your medications (significant other, family member, friend), they are welcome to join for your session.  

Registration is requested in advance to ensure we can offer a private encounter.
