First Baptist Church Hillsboro AWANA Terms and Conditions:

1. I/we understand that my child(ren) may participate in physical activities such as those held during "Game Time". As with any physical activity, there is a risk of injury. I/we fully accept this risk and hold harmless from any legal liability First Baptist Church Hillsboro and any persons involved in the AWANA Club ministry.

2. In the event of an emergency that requires medical treatment of my child, I/we understand every effort will be made to contact me/us or the emergency contact. However, if I/we cannot be reached, I/we give permission to the AWANA volunteers to secure the services of a licensed medical professional(s) to provide the care necessary for my/our child's well being. I/we assume responsibility for all costs connected to any accident or treatment of my/our child(ren).

3. I/we grant permission for a photo of my child(ren) to appear in unpublished club information to be used by First Baptist Church Hillsboro AWANA only. I/we also give permission for photos of my/our child(ren) to appear among other general club photos as long as there is no identifying information shown.

4. I/we grant permission for my/our child(ren) to travel to/from AWANA Club events with an adult leader. Any such event will be clearly communicated with me beforehand.
