Welcome and thank you for stopping by! We appreciate your input on helping to nominate our award winners each year.

This nomination period covers from events and activities from 1 December 2021 through 1 November 2022. Nominations can be made at any time the nomination period is open. We made an extension this past year to include November Activities in the 2021 Awards.

The Nominations are anonymous, unless you choose other wise at the end of the form. Google Forms does capture IP address information for their reporting/recording purposes.
