Harvest Church: Test & Protect Registration
The UK government requires that Churches help the NHS Scotland Test & Protect during the Covid19 pandemic by recording the name and contact details of attendees at its services. This form allows you to register those details.
If you are completing this in advance this indicates that you intend to attend at one of our gatherings.

Symptoms: If you have symptoms of COVID-19 or are self-isolating due to living with someone who is displaying symptoms or as a result of contact tracing then please stay at home and let us know ASAP.  Likewise, if you develop symptoms of COVID-19 during a gathering then you will need to inform us of this right away, after which you should leave the venue to return home to book a test.      Face coverings: It is a legal requirement that face coverings must be worn by all worshippers (aged 5 and over) attending the gathering at all times unless you have a medical exemption from doing so.
