By completing this online form each time you attend an interscholastic competition, you 1.) attest to and acknowledge no personal symptoms of the COVID-19 illness, and 2.) agree to being listed in the game attendance record for any contact tracing needed this season, and to help us monitor the 2 spectators per participant attendance policy. Please note, several schools you visit in the OHSL will not be permitting spectators; please adhere to this position those schools have taken.
