  • Check in begins Thursday, June 8, at 8:30 pm. Grad Night begins at 9 pm, doors close at 10 pm. 
  • Grad night ends Friday, June 9, 3 am.
  • One completed form per Student is required for entry into Redwood Safe and Sober Grad Night 2023.
  • Signatures are required from BOTH the Parent/Guardian and the Student regardless of Student's age.
  • This electronic form serves as Permission Slip and Waiver.
The Redwood Grad Night 2023 Code of Conduct must be read and agreed upon by both Parent/Guardian and Student. Link to the Code of Conduct can be found here

Parent/Guardian and Student verify that Student is physically fit to attend/participate and have fully disclosed below all allergies, medicine or food reactions or unusual physical conditions which could limit participation in Redwood Grad Night 2023. In the case of illness or accident, Parent/Guardian and Student grant permission to the Redwood PTSA to authorize emergency treatment to be administered to Student, including emergency transport, and assume full responsibility and costs involved.

Parent/Guardian and Student Assume Full Responsibility for any risks of loss, or personal injury, including death that may be sustained by Student, and/or any loss or damage to property owned by the Student or by Mt. Tam Racquet Club, as a result of participating in Redwood Grad Night 2023. 

Parent/Guardian and Student agree to Hold Harmless the Redwood PTSA, any Redwood Grad Night 2023 vendors, Mt. Tam Racquet Club ownership and management, the Tamalpais Union High School District, Redwood High School, Tamiscal High School, San Andreas High School and any other volunteer or entity involved in Redwood Grad Night 2023 from any and all liability, claims, actions, demands, expenses, attorney fees, breach of contract actions, breach of statutory duty or other duty of care, warranty, strict liability actions, and causes of action whatsoever.

Acknowledgment of Understanding: The Parent/Guardian and Student have read this Agreement and fully understand its terms. The Parent/Guardian and the Student further acknowledge that they are agreeing to this agreement freely and voluntarily and intend their agreement to be a complete and unconditional release of all liability as set forth in this Agreement.
