By completing the form below, I am electing to participate in the Retirement Program offered by the Ventura Unified School District.  If my participation is accepted by Ventura Unified, then I am tendering my irrevocable retirement from the Ventura Unified School District.  I understand that my form must be received no later than 4:00pm on Wednesday, April 22, 2020, in order to be considered.
