This is an appointment REQUEST, not a scheduled appointment. Please use this form to give all options in your schedule for the week you would like to be seen.  If this is urgent, please also text us at 530-430-1116.  The more options you give us, the more likely we will be able to make it work.  We generally see our furthest from Davis appointments first and then work our way back to Davis.  Days available for the specific week are noted with the date.  If you are a new client, please fill out the New Client Information form as well.  

We have been very busy and may not be able to get to you within the week, we are currently scheduling about 1-2 weeks out.  The schedule for the week is made Sunday. If you are filling this form out after Sunday, please message us to see if we can fit you in.
