Digital Arts Camp Registration
Welcome to the Digital Arts Camp Registration at the Deane Center for the Performing Arts! Please fill out all fields of this form BEFORE paying for your camper(s) class enrollment fee. Campers must be registered by June 25, 2023 to participate. Program acceptance is first come, first serve as there is a limited number of seats available in each camp.

By signing my name below, I hereby give my approval for my child's participation in any and all activities prepared by the Deane Center during the selected camp(s). In exchange for the acceptance of said child's candidacy by the Deane Center, I assume all risk and hazards incidental to the conduct of the activities, and release, absolve and hold harmless the Deane Center for the Performing Arts and all its respective officers, agents, and representatives from any and all liability for injuries to said child/children arising out to traveling to, participating in, or returning from selected camp sessions.

In case of injury to said child, I hereby waive all claims against the Deane Center for the Performing Arts.

I understand that I will have to pay the enrollment fee for each camper (unless granted a scholarship) due by July 3, 2023. I understand that this fee is non-refundable. If I apply for a scholarship for my child/children, I agree to provide the required materials for the application. I understand that there are limited seats and enrollment into the Digital Arts Camp is on a first come, first serve basis.

BY ACKNOWLEDGING AND SIGNING BELOW, I AM DELIVERING AN ELECTRONIC SIGNATURE THAT WILL HAVE THE SAME EFFECT AS AN ORIGINAL MAUNAL PAPER SIGNATURE. THE ELECTRONIC SIGNATURE WILL BE EQUALLY AS BINDING AS AN ORIGINAL MANUAL PAPER SIGNATURE.

I UNDERSTAND THAT THIS CONTRACT IS BINDING AND THAT THE DEANE CENTER FOR THE PERFORMING ARTS DOES NOT ASSUME ANY RESPONSIBILITY OR LIABILITY IN THE EVENT OF AN EMERGENCY OR ACCIDENT. BY ACKNOWLEDGING AND SIGNING BELOW, I AM DELIVERING AN ELECTRONIC SIGNATURE THAT WILL HAVE THE SAME EFFECT AS AN ORIGINAL MAUNAL PAPER SIGNATURE. THE ELECTRONIC SIGNATURE WILL BE EQUALLY AS BINDING AS AN ORIGINAL MANUAL PAPER SIGNATURE.
