Due to COVID-19, student-athletes will be given the option of being transported by school-provided transportation or by a legal parent/guardian.  Student-athletes who choose transportation by a legal parent/ guardian must be transported to and from the event by a legal parent/guardian.  Student-athletes are not permitted to drive to and from events.

The signature below indicates that the parent/guardian relieves Hillsborough Township Public Schools and its employees of all responsibility for transporting the student indicated.  Additionally, transportation for said student to and from the event will be the sole responsibility of the parent/guardian.  Further, the parent/guardian agrees to waive all rights to claims against the Hillsborough Township Public School District and its agents/employees as they relate to this matter.

Finally, submitted response of this form must be printed and shown to the designated coach upon departure from the event to ensure that all student-athletes are accounted for since the coach/bus/team will not be able to leave until all student-athletes, including those not riding the bus, have been accounted for by a staff member or the parent/guardian.  It is also critical that the parent/guardian is on time for pick-up so the bus is not delayed in its departure.
