"Returning" to de Portola (previous student)
In planning for next year's enrollment and accounting for your student's return to de Portola we are asking for parents to complete this form to pre-register your student(s). The information below is needed for any School Choice family who withdrew during the pandemic and plans to return for the 2021-22 school year.

Please fill out the requested information:
 - if your student previously attended de Portola in 2019-20, 2020-21
or
 - if you were granted School Choice acceptance in either of those years, did not attend, but wish to attend now.

We will need this information for each "returning" student. If there is more than one child, please fill out the form for each one. We will need to verify residency and the current school being attended. As such, additional  documents will be needed for enrollment at de Portola. Please cross reference our school's website for other necessary documents.

1. TWO proofs of residency - utility bills ONLY (gas, water or electric)
2. Up to date/Most recent progress report or report card
3. Release of student records completed by current school (1st through 6th graders only). Link available on de Portola's website under "Enrollment".

For incoming FIRST GRADERS only, submit to kristen.gallegos@svusd.org:
- First Grade physical form with stamp and signature, also called "Report of Health Examination for School Entry."

Per SVUSD Student Services, all School Choice families wishing to return to de Portola MUST be present on the first day of school (August 16, 2021).  If a student is not physically present at de Portola on August 16, School Choice to de Portola is forfeited.   *
