Athletic Code Attendance Receipt
Completing the information on this document after attending the parent/coaches meeting for your team sport will fulfill the Co-Curricular Code Requirement for Sevastopol High School and Middle School athletics.

Student-athlete's full name. *
Full names of the parents of the student athlete. *
List grade level of the student athlete and the sport they are participating in (i.e. 9th-cross country). *
Did both the athlete and parent complete all necessary  eligibility paperwork as indicated on the Sevastopol Athletics page? *
Sportsmanship is required to be demonstrated and used by our coaches, players, parents, students, and fans. Who models what sportsmanship looks like at a sporting event? *
High School: At the end of a quarter, students MUST pass ALL classes and earn at least a 1.5 GPA for the previous quarter. If the student has an F in a non-core class the student must have earned at least a 2.5 GPA for the previous quarter. If students do not meet the criteria above, students will not be allowed to participate in games, contests, or performances for 15 consecutive school days.  Students must attend Five - 30 minute study sessions AND show evidence of passing all classes prior to being reinstated. *
Middle School: Must maintain passing grades in core subjects grades will be checked EVERY Friday. Students with a failing grade will be asked to meet with the teacher in the class they are struggling in to formulate a plan to raise the gradeIf the student is still failing the same class on the following Friday, a letter will go home and the student will be ineligible to participate in games, meets, or matches until they have secured a passing gradeWe strongly recommend ineligible students to schedule after-school study sessions (coaches prefer students to arrive late to practice rather than continuing to fail a class) *
A participant must be in school all day on the day in which he/she is to practice or participate in a contest except in cases of being a district approved 100% remote learner, family emergency, excused preplanned absences, or a written medical release obtained from a physician.  (Driver’s license tests are NOT excusable UNLESS a pre-planned absence form is filled out and turned into the office 24 hours prior to the student’s absence.) *
All students involved in co-curricular activities shall not illegally use and/or possess any tobacco product, alcohol, controlled drugs, or drug-related paraphernalia during the entire calendar year (including summer months). *
24 hour rule applies to all parent contact with a coach. No Parent, Player, Coach, or Fan is allowed to engage with an official at the end of the game, contest, meet, or match. *
If a student athlete or player has a concern with game strategy, playing time, or the coach they should follow the proper chain of command.  Athletes/parents should go directly to the coach first. If understandings are not made, then proceed to the athletic director. *
