Please read the following carefully before proceeding to fill the form

You are signing up for the Parent Physics Night for the First Semester. Please note that the Student and the Parent together should be present at the Parent Physics Night for the entire duration of 90 minutes. Please arrive in at least 10 minutes early for Security Check.

Before signing-up the form, please make sure that you will be able spend 100 minutes in all at the Parent Physics Night.
We do not allow partial presence and partial participation. If you cannot make to Parent Physics Night or cannot participate fully, your child will do the original projects assigned to him/her. We do not give credit for partial presence/participation.

Please note that only Parent(s) or Legal Guardian (as found in our school records) are allowed to sign-up and be present for the Parent Physics Night. Parent or Legal Guardian cannot assign someone else to represent them on their behalf for the Parent Physics Night.

For security reasons, the Parent or Legal Guardian  will be required to produce their State ID/Driver's License as a proof for their identity.

For security reasons, the whole program will be recorded on video. Thank you!

The parent and the student should be present  for the entire duration of 90 minutes of the Parent Physics Night and they need to arrive at least 10 minutes earlier for security check. Do you agree to this? (Those who cannot meet this requirement will not earn the exemption from two Final Exam Tasks. They will be asked to complete and turn in the original projects. We do not give partial credits.) *
