In order for staff, students and families to be as safe as possible, it is important that we carry out lateral flow tests at Eastlea School. This will help us to slow down the spread of Covid-19. Students will carry out three tests in school over a two week period and will then be given tests to take home from that point.

Please read the points below and then if you agree to them, please complete the information on the next page.

- I consent to my child having nose/throat swabs for lateral flow tests (self swab).
- I understand that my child’s sample will be tested for the presence of COVID-19.
- I understand that if my child’s results are negative on the lateral flow test I will not be contacted by the school.
- If the lateral flow test carried out on site indicates the presence of COVID-19, I understand that my child will
need to self-isolate for ten days.
-  If the lateral flow test carried out at home indicates the presence of COVID-19, I understand that my child will need to self-isolate until they have a PCR test and can only return if the results are negative or following isolation. I will report this to the school immediately and I understand that my child will be required to self-isolate following public health advice.

PCR* tests are the swabs that are returned to a laboratory for analysis. The results normally take 24 - 48 hours. To book a PCR test please call 119.

REMEMBER IF YOUR CHILD IS SHOWING SYMPTOMS OF COVID-19, THEY MUST NOT COME TO SCHOOL UNTIL THEY HAVE HAD A TEST. A TEST CAN BE BOOKED BY CALLING 119 FOR GUIDANCE.
