Crews Lake Middle School New Student Course Requests (2021/2022)
This form is to be completed by parents and students who are transferring to Crews Lake Middle School during the summer. School Counselors and Administration will use these requests to guide their scheduling of the student. In all cases students are scheduled using the Student Progression Plan and requirements for promotion from middle school to high school as the basis for scheduling.

Middle School Promotion Requirements: All Pasco County middle school students must COMPLETE and PASS the following middle school level courses to be eligible for promotion to 9th grade: 3 years of English Language Arts, 3 years of Mathematics, 3 years of Science, and 3 years of Social Studies which must include at least 1 semester of Civics curriculum.

Students who have previously been enrolled in a Pasco County public school, including Prekindergarten, will retain their ID number throughout their enrollment in Pasco Schools. If the student has never been enrolled in a Pasco County school, do not complete this field.

If the student was enrolled and registered in Home Education in Pasco County, please list Home Ed as the previous school. Parents must contact the Home Education Office to close the Home Education program before the student can be enrolled at CLMS. Notification can be sent to HomeEd@pasco.k12.fl.us. You should contact the School Counselor regarding grades earned during the home education program.
