If you wish to freeze or cancel your MSS Swim Team account please fill out the following form.
Forms must be submitted by the 1st of the month.
Per sign up agreement: "Failure to submit a cancellation form no later than the 1st of the month will result in a charge equal to 50% of your monthly due."
**Accounts with a balance can not be frozen/ cancelled until the balance is paid in full.
*If you have multiple swimmers attached to your account but are not freezing all of them, please only include the names of the swimmers that will no longer be swimming.
