The undersigned hereby voluntarily and knowingly waives and releases any and all claims for liability, injury or damages that may arise out of my participation in the activities organized by Hope is Here Counseling & Consulting Services LLC & A Lot Like Living LLC (“Hope & Living ”), two Ohio limited liability companies, at Blue Ash Library, 4911 Cooper Rd. Blue Ash, OH. 45242 & The Potted Palm, 2123 Beechmont Ave., Cincinnati, OH 45230. This waiver and release shall include any claims I may have against Hope & Living and all members, owners, trustees, officers, directors, employees, volunteers or other agents or representatives of said company. I acknowledge that I understand this is not a therapeutic group and my participation does not enroll me in counseling services with Hope & Living. I voluntarily assume the risk of any injury as a result of my participation in activities. In the event I need immediate medical attention while participating in the retreat, I hereby authorize Hope & Living to take all reasonable steps to obtain such medical attention. Further, I authorize Hope & Living to use any photographs, images, videos and audio recordings of me for promotional purposes, including brochures, websites, posters and displays.
