Please answer the following questions in a separate form for each of your children, grades 7-12. This is for second quarter, which runs October 26-January 15. Surveys must be returned by Wednesday, September 21st. Data will be used to plan schedules and allocate personnel for second quarter. It is IMPERATIVE that you return the survey to allow us to plan accordingly. A separate survey will be sent at a later date for younger students. Thank you!!
