Bethel-Tate Local School District is in the process of planning for second-semester learning. To help with our planning, we recently sent a survey to our families to give us insight into the number of households interested in changing learning models.

This current form is to select your child's learning model for the second semester which begins on January 4, 2021. Your child must remain in this selected learning model for the entire second semester.

The district will continue to offer two choices for learning:
1. In-person learning (five full days a week)
2. On-line virtual learning academy (full remote)

You must fill out this commitment form even if your child is staying in the same learning model for the second semester. Please fill out a separate form for each child. This commitment form must be submitted by 5 p.m. on Friday, November 13, 2020.
