5th Grade Ukulele Checkout 22-23

Please fill out this form if you would like to check out a ukulele for the 22-23 school year. Please read through the entire form and submit. Checked out ukes will come home on October 17th and should stay at home until the end of the school year.

Lake students are the only family members that should be using the ukuleles

Practicing is CRUCIAL! Please practice at least 15 minutes a day. Tracks and music can be found on Mrs. Corso's 5th Grade Google Classroom.

Please take care of the ukulele. Families are responsible for the checked out ukulele and will need to pay $60 to replace.

If you find that your student is not practicing at home, please send the ukulele back to school to be checked in.

This ukulele stays AT HOME. No need to bring to school until the end of the school year when we will check ukuleles in.
