TOT Ball Registration Form (MHPA)
This form will walk you through the registration process for the Wednesday 12:00 pm - 12:45 pm class held at Mars Hill Preschool Academy.  Please complete this form and submit payment to your child's teacher in the form of check made out to MHPA and write "TOT Ball" in the memo line.  

Cost:
One-time $35 Registration Fee (due by first class)
Monthly $50 Tuition (due by the first class of each month)

Weekly Classes held September 1 - April 27.

Minimum Enrollment: 8
Maximum Enrollment: 30 (Classes over 15 students will have licensed and certified TOT assistant)

PICK UP:
Classes held in the Mars Hill Baptist Church Gym.  You may pick up your child on Wednesdays via the car line BEHIND the gym at 12:45 pm. Please be on time and notify Stephanie (229-854-0141) if you have an issue.

Note: Students are not fed lunch during TOT Class. We do have the occasional treat at the end of class.  (Please see form below to enter any food allergy info.) Most parents choose to send lunch in a lunch box and just have their child eat in the car on the way home from school.
