Request Form - Clear Creek CPL/TPL/OSSL Game
Use this form to request to add or remove a CPL, TPL, or OSSL game at Clear Creek. In order to decrease the likelihood that your request is modified or rejected, you can check the Field Availability sheet to find a date and time that's probably available: https://bit.ly/ClearCreekCPL.

Requests should be submitted as early as possible and at least 7 days in advance. Requests involve work for the club admins, field assignors, ref assignors, and both teams.

  • Adding a game will ensure that you'll have a field reserved and referees assigned. Make sure your opponent has agreed to the day and time you're requesting first.
  • Removing a game will ensure that the assigned referees know the game is canceled and cancel the field reservation.
  • Rescheduling a game requires you to submit the form twice.
*For OSSL games, this form only covers the field reservation - updating the referee schedule requires an additional step on the OSSL website.
*You should update the CPL league website, your opponent, and your team calendar, once the requests are approved.
