By Library policy and State Code (Sec. 53-21a) children under the age of twelve cannot be in the library without a parent or guardian.

The following form provides permission for your child to attend an after-school program at the library without a parent or guardian present. It dually grants permission for our staff member to escort your child from Booth Free School to the Minor Memorial Library premises.
