Northwest Local School District Family Survey - FIRST SEMESTER FEEDBACK
Please respond to the following questions to guide our district and building leaders as we plan for the second semester. Our intent is to gather feedback about our current model and to look at options for revisions for second semester. This survey is for both remote and in-person families. Only one response is requested per family. We request that this survey be completed by Friday, November 13th.

**Please note: We will be sending a second survey out in November to allow you to request your preferred placement (remote or in-person) for the second semester, which begins on January 20th. Prior to asking for a decision, all families will be notified of any changes that we plan to make to the remote and/or in-person learning models. Our ability to honor requests for returns to in-person learning may be limited based on decreases in classroom capacity as a result of social distancing guidelines.
