St Joachim's Child Absence Reporting
Absence and Late Reporting. 
To report your child's absence from school today - complete this form before 9.00am each day your child is off school. Please ensure you fill out this form accurately and to submit the form each day your child is absent in order to keep the school and class teacher informed.
If a child is on medication for their illness, parent must bring the medication to the school office where they will complete a permission slip for school to administer medication. If this is not followed, school will not be able to administer the medication.
For health and safety reason, please do not send your child with medication.
