Please fill out this form in order to request an activity for your club. ASB will contact you once your activity has been approved or not, IF NOT please come to room 16 BEFORE proceeding.

Also, your activity has not been approved until you get a note from ASB letting you know, so please send in requests as early as possible (recommended 1-2 weeks in advance).
