Registration Form for Summer 2022 Sports Camp

GRADES: Entering grades 3rd - 8th for the 2022-2023 school year

DATES: July 25-29
TIME:  3rd-5th grade, 8:00AM - 12:30PM; 6th-8th grade, 12:30PM - 4:00PM
LOCATION: NCHS

DETAILS: 3rd-5th grade will participate in a different sport each day.
Monday: Volleyball
Tuesday: Flag Football
Wednesday: Basketball
Thursday: Soccer
Friday: Track & Field

6th-8th grade will choose one sport to participate in throughout the week. Options for 6th-8th grade are Basketball, Flag Football, and Volleyball.

COST: $75 for camp, $90 for camp and T-shirt; $100 for camp, T-shirt and provided lunch (Link for payment will be sent out after registration is completed. You will also be sent an email with lunch options.)

PRE-REGISTERATION DUE:  July 18th

* NCS 2022 Summer Permission Form,  required one time for all programs, activities and trips on and off the campus of Northwest Christian Schools of Lacey for the summer of 2022. Permission form link will be provided after submission or can be found at https://ncslacey.org/athletics-nchs/

Please list all those able to pick up your student from the Sports Camp. Only parent/guardians and those on the pick up list will be permitted to pick up your student.

I, the legal parent/guardian, commit to pay all financial obligations on or before due date. I understand that all payments will be made via Square through a link emailed after the completion of registration. LATE PICK UP FEE - If I do not pick up my child by 12:30pm (3rd-5th) or 4:00 (6th-8th), a late fee will be incurred. By signing your name below, you agree to these terms and conditions.
