Community Well-Being Self/Community Connection to Services  
The Community Well Being Ambassadors will provide support to youth, families and other community members who may be referred by other organizations, groups, the community at large or may be self identified.

Ambassadors are trained in helping individuals identify their most pressing needs, providing relevant information, and developing strategies for addressing those needs, including connection to behavioral health and other community services and ongoing support.

The Community Well-being Initiative (CWBI) is a partnership between the University of Delaware, Delaware Division of Substance Abuse and Mental Health (DSAMH), Network Connect (NC), Center for Structural Equity/CIT (CFSE), Minds in Motion (MiM), GBA Consulting, and community-based host sites. Funding for this pilot project is supported by DSAMH. We want to know how well this project helps community members. The project is collecting some information about people.  Partners outside of NC, CFSE, and MiM  will not see any names of people served by the CWBI. Only information like type of referrals will be shared. Signing this form means your information will be shared to evaluate the project.
