This Release and Waiver of Liability ( “Release”) executed on THE DATE OF SUBMISSION OF THIS FORM is and between THE PARENT(S) OF THE REGISTERED MINOR STUDENT-ATHLETE ACTING ON BEHALF OF THEMSELVES AND THEIR MINOR STUDENT-ATHLETE (“Releasor”) and Decrypt Coaching LLC (DCLLC). The Releasor contemplated herein includes but is not limited to the following: Student-athlete, participant, parent, stepparent, guardian, or host family. Releasor understands that he/she is responsible for his/her own insurance coverage on behalf of their student-athlete in the event of personal injury or illness as a result of his/her participation with Beginner Field Hockey Clinic or any other version/rendering of the clinic name.

1. Waiver and Release: I, the Releasor, hereby release and forever discharge and hold harmless DCLLC and its successors and assigns from any and all liability, claims, and demands of whatever kind of nature, either in law or in equity, which arises or may hereafter arise from the services I provide to or participation with DCLLC whether for a specific event or otherwise. I understand and acknowledge that this Release discharges DCLLC from any liability or claim that I may have against DCLLC with respect to any claims, including but not limited to bodily injury, personal injury, illness, death, or property damage that may result from the services I provide to or participation with DCLLC whether for a specific event or otherwise.

2. COVID-19: I, the Releasor, hereby agree and understand that DCLLC strives to act at all times in a manner consistent with the guidelines promulgated by the relevant leading research bodies and governmental agencies, thereby doing everything we can to protect the Releasor, our staff, and the public at large.

I ATTEST THAT, IF MY CHILD IS FEELING SICK, UNWELL, HAS A FEVER, IS EXHIBITING ANY SYMPTOMS ASSOCIATED WITH COVID-19, OR HAS BEEN IN CONTACT WITH ANYONE RECENTLY DIAGNOSED WITH COVID-19, OR ANY OTHER CONTAGIOUS ILLNESS, I WILL KEEP HIM/HER/THEM HOME FROM THE BEGINNER FIELD HOCKEY CLINIC.

3. Insurance: I understand that DCLLC does not assume any responsibility for or obligation to provide me with financial or other assistance, including but not limited to medical, health, or disability benefits or insurance. In accordance with paragraph 1 above, I expressly waive any such claim for compensation or liability on the part of DCLLC. By my signature below, if I am signing on behalf of a child, I hereby agree that my child has a valid health insurance policy in place.

4. Medical Treatment: I hereby Release and forever discharge DCLLC from any claim whatsoever which arises or may hereafter arise on account of any first-aid treatment or other medical services rendered in connection with an emergency during my time spent with DCLLC. If I am signing on behalf of a minor child, by signing below I verify my child is in good health and has my permission to participate in the beginner field hockey clinic. I authorize the DCLLC staff and volunteers to act for me in securing medical treatment for my child in the event of injury or sickness. I understand that injuries occur in sports, and in accordance with paragraph 1 above, I expressly waive any such claim for compensation or liability on the part of DCLLC.

5. Assumption of Risk: I understand I may be exposed to activities that may be hazardous to me. As a Releasor, I hereby expressly assume this risk of injury or harm from these activities and release DCLLC from all liability in accordance with paragraph 1 above.

6. Photographic Release: I grant and convey to DCLLC all rights, titles, and interests in any and all photographs, images, video, or audio recordings of me or my likeness or voice made by DCLLC in connection with my participation with the beginner field hockey clinic. I hereby grant DCLLC permission to use such photos or videos while I participate in the beginner field hockey clinic, for promotional purposes. As a client on behalf of my minor child, I hereby grant DCLLC coaches, staff, and volunteers to use via all forms of social media, U.S. mail, and email permission to use such photos or videos while my child participates in the beginner field hockey clinic, for promotional purposes, so long as my child’s name is not affiliated with the photo or video.

7. Other: As a Releasor, I expressly agree that this Release is intended to be as broad and inclusive as permitted by the laws of the State of Maryland and that this Release shall be governed by and interpreted in accordance with the laws of the State of Maryland. I agree that in the event that any clause or provision of this Release is deemed invalid, the enforceability of the remaining provisions of this Release shall not be affected.

BY E-SIGNING BELOW, I EXPRESS MY UNDERSTANDING AND INTENT TO ENTER INTO THIS RELEASE AND WAIVER OF LIABILITY FOR DCLLC, WILLINGLY AND VOLUNTARILY, AND ON BEHALF OF MY MINOR CHILD.
