Portrait Pioneers Award Program Nomination Form
In the spirit of our school district’s ongoing focus to inspire, prepare and engage all Plainville students to be college and / or career ready, the Plainville Community Schools “Portrait of the Graduate” was developed to define and outline the critical skills necessary to be successful upon graduation. Students who embody the Plainville Community Schools Portrait of the Graduate model are: Collaborative, Communicative, Innovative, Mindful, and Resilient.

To reinforce the key Portrait of the Graduate characteristics, and to recognize school district faculty and staff members who demonstrate and model these traits for our students, we have developed the “Portrait Pioneers” recognition program. Similar to the former “Everyday Hero” recognition program, school district employees can be nominated by colleagues, students, parents, or community members as Portrait Pioneers, and will be recognized at Board of Education meetings, in district communications, and on the school district website.

If you would like to nominate a member of our school district community who you believe is a “Portrait Pioneer” please complete this form.

Note: Nominations may be posted in their entirety or in summary to the district website. If you wish to have your nomination kept confidential, please make note of that in your nomination response below. The nominator and nominee will also be invited to attend the Board of Education meeting for these recognitions, whether virtual or in-person. Thank you!

With much appreciation,
Steve LePage, Superintendent of Schools
