FSL Game schedule request
Use this for to request pre-play and makeup games.  

Be sure to follow these guidelines to ensure your request is granted in a timely manner.

1. Work it out with opposing coach for 3 possible dates and time
2. Check the schedule and be sure date and time is open
3. Requested date and time should be:
         a. First choice during your practice time
         b. Friday or Saturday if space available
4. Select the time requested for your game Eg:  Weekday Early Game (@6PM +/-)
5. If you do not see your game listed in drop down, choose :
     
        Match Not Found - See Notes (Include Match #, Date and Opponent)

6. Once form is completed you can follow the progress of your request here:

7. Do not inform opposing team of any date until updated in gotsoccer
