I recognize the possibility of physical injury associated with soccer, and hereby give my consent to have an athletic trainer, coach, team manager, emergency medical technician, nurse, medical treatment facility, and/or doctor of medicine or dentistry or associated personnel provide the applicant/participant with medical assistance and/or treatment and agree to be financially responsible for the cost of such assistance and/or treatment. I understand treatment for injury will be based on information provided herein. I hereby authorize emergency transportation of the applicant/participant to a medical treatment facility should an individual listed above consider it to be warranted. I, for myself and the player and our respective heirs, administrators and successors, intending to be legally bound, hereby release and indemnify Oakland Soccer Club, and their respective directors, officers, employees, agents, representatives, the owners and operators or the facilities used for the programs from and against all claims, liabilities, damages or causes of action arising out of or in connection with the player’s participation in the Programs including, without limitation, player’s transportation to/from any Program, which transportation is hereby authorized. I further grant the Oakland Soccer Club Parties the right to use the player's name, picture and/or likeness in printed, broadcast and other material concerning the Programs provided such use is related to the player’s status as a participant in the Programs. - : - Reconozco la posibilidad de lesiones físicas asociadas con el fútbol y, por la presente, doy mi consentimiento para que un entrenador de atletismo, entrenador, gerente de equipo, técnico de emergencias médicas, enfermera, centro de tratamiento médico y/o doctor en medicina u odontología o personal asociado brinde la solicitante/participante con asistencia médica y/o tratamiento y acepta ser financieramente responsable por el costo de dicha asistencia y/o tratamiento. Entiendo que el tratamiento de la lesión se basará en la información proporcionada en este documento. Por la presente autorizo el transporte de emergencia del solicitante/participante a un centro de tratamiento médico en caso de que una persona mencionada anteriormente lo considere justificado. e indemnizar a Oakland Soccer Club, y sus respectivos directores, funcionarios, empleados, agentes, representantes, propietarios y operadores o las instalaciones utilizadas para los programas de y contra todos los reclamos, responsabilidades, daños o causas de acción que surjan de o en conexión con la participación del jugador en los Programas, incluido, entre otros, el transporte del jugador hacia/desde cualquier Programa, cuyo transporte se autoriza por el presente. Además, otorgo a las Partes del Oakland Soccer Club el derecho a usar el nombre, la imagen y/o la imagen del jugador en material impreso, transmitido y de otro tipo relacionado con los Programas, siempre que dicho uso esté relacionado con el estado del jugador como participante en los Programas. *
