Legacy Leadership Team will not be held responsible for any injuries or illnesses that my child sustains during the activity.

I hereby authorize adult Legacy leaders, as agents of myself, to provide routine health care, and to seek emergency medical treatment, if deemed necessary. In the event that I cannot be contacted during an emergency, I authorize the physician or hospital selected by the leaders of Legacy to provide treatment, including hospitalization, for my child(ren).
